Why do we keep looking at the same problems?

In their roadmaps, there are problems they are repeatedly trying to solve and they are reluctant to question the status quo.
Of course, this is normal for at least two reasons:
The unknown is risky and our brain is programmed to stay safe.
The education systems rewarded us for answering (rather than asking) questions.
However, I have thousands of anecdotes where in businesses you’re more rewarded to step back, explore the unknown, and ask the right questions.
